Register for Growing Green Leaders

This form follows Rotary International’s youth protection guidance for programs that involve minors, including required written parent or guardian permission. It takes about five minutes.

Youth protection at Growing Green Leaders. In line with Rotary International's Statement of Conduct for Working with Youth, every participant under 18 must have a parent or legal guardian complete the permission and consent sections of this form. Rotary volunteers who work directly with youth in this program have completed Rotary's required youth-protection screening. Any concern about a young person's safety can be reported using the contact information at the bottom of this page.
1. About the participant

Participants under 18 need a parent or legal guardian to complete Section 3 of this form.

2. Parent / legal guardian information

Required for all participants under 18. If the participant is 18 or older, you may skip this section.

3. Educator or mentor (optional)

If a teacher, club advisor, or Rotary mentor is coordinating this registration on behalf of a classroom or club, add their details here.

4. Permissions & consent
For participants under 18: these boxes must be reviewed and checked by the parent or legal guardian named in Section 2, in accordance with Rotary International's youth-protection requirement that parents or legal guardians give written permission before a minor participates in a Rotary-affiliated youth program.

If the participant is under 18, this signature must be entered by the parent or legal guardian named in Section 2.

Data privacy. Information collected on this form is used only to administer Growing Green Leaders registration, mentorship matching, judging, and required safety communication with parents, guardians, educators, and Rotary club contacts. It is not sold or used for unrelated marketing. Questions about how information is stored or a request to update or remove a participant's information can be sent to our contact page.
